Introduction

In view of the rising food expenses, St. James’ Settlement People’s Food Bank introduced “Philanthropic Nutrition Programme” on 1 July 2016 to support grassroots, low-income families and elderly in poverty, providing them with adequate nutrition for better health.

Service Targets

  • Infants, children and elderly from low-income families (in accordance with 55% of Median Monthly Domestic Household Income by Household Size)
  • Infants, children and elderly from families receiving CSSA

Application Procedures

Eligible applicants please call or go to service centres in person to make appointment with social workers, or through referral organisations to submit applications. Applicants are required to submit application form (must be filled in by social workers) and provide identification, residential documents, income and assets proof, etc., of the applicant and household members for eligibility assessment.

Remarks

  • This is a charity project, and the milk powder is mainly by donation.

     – Infants from new-born to 6 months old, SJS will arrange the usual brands of milk powder but subject to the stock.

     – Infants or children from 6 months to 6 years old, the milk powder distributed is subject to donations in that period and cannot be chosen.

  • Income and asset assessment are required for application.
  • Service period: consecutive 6 months at most. (One-off application only, no extension or reapplication is accepted.)
  • One-off application for each child/ elderly recipient, no extension or reapplication is accepted.
  • Applicants who were beneficiaries of “Children Milk Powder Sponsor Scheme” or “Nutrition Pack for the Elderly” are not entertained for the programme.
  • Service users of “Philanthropic Nutrition Programme” shall not apply for “Short-term Food Assistance” at the same time. Once the circumstance occurs, service provided to the user will be terminated immediately and no reapplication will be accepted.
  • St. James’ Settlement People’s Food Bank shall exercise the right of application approval.

Required Documents

Prior to handing in the application form, referral workers should fill in the form and remind the applicant to prepare the following documents:

Personal Identity Information

Hong Kong Identity Card/ identification documents or birth certificates of applicant and that of his/ her family member(s)

Remark: If applicants cannot provide identity cards, please provide other documents that help to prove the applicants’ identity.

Residency Information

Address proof in recent three months (e.g. Copy of tenancy agreement, electricity bill, water bill, etc.)

Household Income Information

Proof of household income in recent three months for the applicant and family members, for example, pay slip, Employer’s Return of Remuneration and Pensions or other types of income proof. If applicants have left the company, please provide proof of resignation.

Household Asset Information

  • All bank books/ bank statements of the applicant and family members with the updated balance within 7 days; a copy of the first page of the bank book showing the name of the account holder and account number, and that of the pages showing the transactions of recent three months (including the cancelled accounts within recent three months).
    * If the bank account is newly opened, please provide the first page of bank book with the name of account holder and account number, as well as the page with the initial balance.
  • Time deposit balance/ statement in recent three months of the applicant and family members
  • All investments of the applicant and family members, with documents showing the cash value and dividend (e.g. Anniversary Statement, Insurance Policy Quarterly Statement, Stocks, Bonds, Funds etc.)
  • Proof of other assets, including properties, parking spaces and bank accounts outside Hong Kong

* All cases will be assessed by social workers of People’s Food Bank. Service will be provided to the eligible applicants.
